Injuries on Duty: Required Documents

Employer's Report of Accident (WCL2)
First Medical Report (WCL4)
Progress Medical Reports (if any)
Identity Document
Proof of Employment and Latest Payslip

Our consultation room is located on Mediclinic’s grounds, in Annex Building 117, Room 2, right behind the main hospital. Whether you’re visiting for your initial injury treatment, follow-up care, or a final consultation, our team is ready to support you at every stage of your recovery. Our partnerships with the Department of Labour's Compensation Commissioner, RMA, FEM, and Coidlink streamline the claims process, allowing employers and patients to focus on recovery without the stress of administrative tasks.
